When you attend the Practice to register you will be asked to complete a GP registration form, and a new patient questionnaire form.
Acceptance on to the list is as at the discretion of the Doctor. We will ask you for proof of identity. (Please refer to our registration policy below)
There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.
Your medical records may take several weeks to arrive at the Practice.
We have an open list and welcome requests for registration from patients living in or moving to the Practice boundary area.

We are welcoming new patient registrations now
You can register by completing the new patient registration form and the new patient questionnaire in full, which are available from reception and also in the Download forms section below.(Download, complete & save the forms, then use the ‘Upload Documents’ section to submit).
After you have completed these forms, You will need to bring in photo identification and proof of up to date proof of address in order for us to complete the registration process.

Updating Your Details
If you change name, address or telephone number, please let us know by writing to us or filling in a form at reception. If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
